Better Birth Outcomes Comprehensive (BBO-C) Program

The Better Birth Outcomes Comprehensive (BBO-C) Program supports pregnant individuals and families through personalized, one-on-one care and guidance. Our Nurse Navigators and Non-Nurse Navigators work closely with clients to provide comprehensive nursing assessments, connect families with community resources, and encourage ongoing engagement with healthcare services and a medical home.

Through individualized support, the BBO-C Program helps families achieve their health and wellness goals while promoting healthier pregnancies, healthier babies, and improved maternal and infant outcomes.

Our Team Can Help You With:

• Understanding and maximizing your medical insurance benefits
• Referrals for prenatal care and assistance scheduling OB appointments
• Referrals to pediatric healthcare providers and infant well-child visits
• In-office and/or home visits with a Nurse Navigator
• Education on topics such as:
• Nutrition
• Pregnancy
• Infant and child growth and development
• Preventive healthcare services
• Referrals for medical care and immunizations
• Clinical visits that may include:
• Height and weight checks
• Blood pressure monitoring
• Depression screenings
• Developmental screenings
• Referrals to community resources for family needs, including:
• Food
• Clothing
• Housing and shelter
• Childcare
• Transportation
• Other supportive services
